Cygnet Hospital Wyke:
Cygnet Hospital Wyke provides a comprehensive pathway of acute admissions services for men. Our team works closely and collaboratively with service users, their families and the referrer to provide a seamless pathway from referral to discharge.
Key Responsibilities

Contribute to assessment, formulation, and implementation of individual care and treatment plans

Assess and address patients’ social circumstances, including housing, benefits, family relationships, and community support
-
Lead on discharge planning to ensure safe, timely, and sustainable transitions back to the community
-
Maintain active liaison with community mental health teams, local authorities, housing providers, and other external agencies
-
Attend and contribute to Mental Health Review Tribunals and other statutory meetings
-
Lead safeguarding processes and respond to safeguarding concerns relating to patients
-
Act as an advocate for patients’ rights, dignity, and social inclusion
-
Complete risk assessments relating to social and environmental factors
-
Provide psycho-social interventions to support recovery and independence
-
Maintain accurate, timely clinical records and reports in line with legal and organisational standards

Support family involvement and facilitate communication between patients and carers where appropriate, cares lead.

Participate in case conferences, ward rounds, and care programme approach (CPA) meetings

Contribute to service development and quality improvement initiatives

Provide support and supervision to the team and students.
Person Specification
-
Qualified Social Worker with current professional registration
-
Minimum of 2 years’ experience working within mental health settings
-
Experience working in inpatient or acute mental health environments
-
Strong knowledge of mental health legislation, safeguarding procedures, and social care frameworks
-
Experience with discharge planning and multi-agency collaboration
-
Ability to manage complex caseloads and competing priorities
-
Excellent communication and report-writing skills

Desirable:
-
Experience working with women in mental health services
-
Experience attending tribunals or court-related processes
-
Training in trauma-informed practice
-
Knowledge of housing and welfare systems.
