We are looking for a confident Physical Health Assistant with a passion for delivering outstanding care. You’ll be working 40 hours a week, Monday to Friday,, making a positive difference to the lives of the people in our care at Cygnet Joyce Parker Hospital. Cygnet Joyce Parker Hospital is our emergency mental health hospital in Coventry. The service supports adults across three wards to move as quickly and safely as possible to the most appropriate and least restrictive environment for them.

You will support the Physical Health Lead Nurse to ensure that physical health needs for service users are assessed and clearly planned and monitor the effectiveness of the actions being taken to address identified needs. You will work with the dietician, OTs and medical and nursing staff to ensure a comprehensive care delivery of physical health needs. The post holder will support patients to access health services in the community and support external professionals within the hospital (ie GP clinic/podiatry services).

A key requirement of the role will be to work alongside staff in a coaching role as well as provide on-going training and education to staff within scope of practice.

Your day-to-day…

  • To support evidence based physical health practice initiatives across the service.
  • Support with the monitoring and documentation of physical health screening across the service.
  • Support with the monitoring and documentation for ongoing monitoring of physical health of all patients including annual health checks in close collaboration with the ward doctors.
  • To work closely with the ward doctors and physical health lead in ensuring that patients across the service have access to primary care services including dental and eye checks.
  • To work in line with current legislation, company policies, and other relevant local & national guidance pertaining to physical health guidance within mental health services.
  • Contribute, as required, to national audits, and support with carry out clinical audits as required, to ensure that standards are maintained in line with Company policy and national best practice. Ensure information and comprehensive records relating to physical health is recorded in MyPath and entered appropriately on other data records as required. 
  • Support the PHN to monitor medical equipment and ensure it is available across site to ensure that patient’s physical health needs are adequately monitored.
  • To support with the delivery of training to registered and non-registered staff.
  • To support the delivery of patient physical health education.
  • To support the lead PHN with the management of infection control procedures within the hospital in accordance with the Company Policies and Procedures.
