We need a Registered Social Worker who’s passionate about helping people with a range of complex mental health needs.

You will be joining our team on a full time, 37.5hrs per week basis.

Job Purpose:

This role requires social workers to meet all the fundamental professional standards as set out by Social Work England and ensure that the protection of children and vulnerable adult is adhered to ensure that children and vulnerable adults are safe and their outcomes enhanced. To respond to the needs of adults with mental disorder and their carers in a timely manner, within the boundaries of statutory legislation, and with regard to individual circumstances. To assess need using a person centred approach and effectively asses and manage risk engaging the framework of the Care Programme Approach whilst working alongside services, family/carers and community services. 

Job Description:  

This job description is not a complete list of duties but gives a general indication of the range of work undertaken. It will vary over time as demands and priorities change. The management team structures with a high degree of collaboration and flexibility, and post holders are expected to undertake any other duties commensurate with the general level of responsibility of the post.

  • To liaise with family and carers to support patients with their recovery. 
    • To be aware of available community services and functions such as Carer’ Assessment and signpost or refer as applicable
  • To be able to manage competing tasks and prioritise work effectively.
  • To work in multi-disciplinary settings including participating in multi-disciplinary assessments, risk assessments, care plans and risk management plans.
  • To engage with patients to complete initial holistic assessments to inform their care, treatment and recovery. 
    • To work with guidelines and policy of the CPA, Mental Health Act, Mental Capacity Act, housing and benefits legislation, Personalisation agenda, Children’s Act, The Care Act 2014 and any other appropriate legislation to establish individual programmes of care.
  • To liaise with external agencies to collect collateral information and engage services to support patient’s discharge pathways
  • To work autonomously, act independently and to make effective decisions whilst managing your own case load.
    • Manage a diverse and complex individual or team caseload of service users and carers
  • You will expected to continuously improve your practice, develop and maintain skills, knowledge and competence by developing your knowledge and skills through post qualifying training.
    • You must be registered to practice with Social Work England under the terms of the code of conduct for social workers.
    • You will be expected to keep up to date with mandatory training as set by Cygnet Health.
  • To provide a service in line with statutory requirements and departmental priorities.
  • Ensure and demonstrate the service users’ and families/carers are listened to and treated with respect with their views recorded in the process of assessments, planning, intervention and review, and in all aspects of decision making and in shaping service delivery.
  • Demonstrate a good understanding of evidence-based practice in your day to day work.
  • A good knowledge of relevant legislation and procedures.
  • Possess strong Safeguarding Knowledge at Level 3 Children & Adults.
    • To identify and action Safeguarding Adult and Children alerts and undertake investigations, attending and presenting at safeguarding meetings and ensuring accurate recording in line with departmental procedures, referring to more senior practitioners for professional advice (including the Safeguarding Leads).
    • All employees must carry out their responsibilities in such a way as to minimise risk of harm to children, young people and adults and to safeguard and promote their welfare in accordance with current legislation, statutory guidance and Cygnet policies and procedures.
    • Employees should undertake safeguarding training and receive safeguarding supervision appropriate to their role.
  • If appropriate to team setting, provide social circumstances reports to the Mental Health Review Tribunal and Hospital Managers as required.
  • Participate positively in professional supervision and annual appraisal review, taking personal responsibility for the quality of practice.
  • Actively contribute to team working, supporting colleagues, and taking part in team meetings and development events.
  • Demonstrate a high level of competence and confidence in the use of Cygnet Health electronic case recording/reporting tool, maintaining accurate and up to date records.
    • To communicate both orally and in writing in a clear, confident and assertive manner in line with established policies, practices and priorities of the service.
  • To be active in implementing Cygnet’s equal opportunities policy in professional practice and service delivery, including recognising the importance inclusion and diversity.
  • Participate in service development and change management where appropriate
  • Operate within legal, ethical and practice boundaries when working with services users and families.
  • Be responsible for Health and Safety and Safeguarding of all service uses with whom you work.
  • Ability and willingness to deputise for line manager when appropriate and requested.
  • To share expertise and knowledge within the team and if interested develop supervisory skills in order to provide guidance, advice, direction and supervision regarding casework and provision of services. This can be with a student, support workers, or less experienced colleagues.
  • Comply with legal obligation (DATA PROTECTION ACT 1984) not to use or disclose any personal information that comes into their possession in the course of their duties in any unauthorised manner. Duties and obligations under the Act that relate to this particular post will be explained to the post holder upon appointment to this post.
